Can I get a diagnosis removed from my medical record? ›

If you think the information in your medical or billing record is incorrect, you can request a change, or amendment, to your record. The health care provider or health plan must respond to your request. If it created the information, it must amend inaccurate or incomplete information.

Once you identify something you want to change, contact your healthcare provider and request a form for making amendments. Be clear with your request. Upon receiving it, your provider will have 60 days to act on your request. Your provider is not required to make the requested change.

Information Included in Medical Records

Typical medical records include: Patient identification, contact information, and date of birth. Billing and health insurance details. List of current and chronic ailments and diagnoses.

The HIPAA Privacy Rule provides individuals with the right to inspect their PHI held in a designated record set, either in addition to obtaining copies or in lieu thereof, and requires covered entities to arrange with the individual for a convenient time and place to inspect the PHI.

How can I correct an error in my records? The patient, including minors, can write an "Addendum" to be placed in their medical file. The original information will not be removed, but the new information, signed and dated by the patient, will be placed in the file.
